PHYSICAL THERAPIST

Dogwood Village of Orange County is recruiting a part-time PT who can provide holistic assessments, management, and treatments for our adult skilled and long-term care residents who have musculoskeletal and neurological disorders.

POSITION SUMMARY

  • PT shall screen, evaluate, re-evaluate, discharge plan and make revisions in any established plan of care as supervised by the physician.

ESSENTIAL FUNCTIONS:

  • Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual.

  • Re-assess patients and caseload as needed in order to determine need for continued services or discontinuation of services based on patient progress.  Carry out treatment plan as indicated.

  • Supervise treatment implementation carried out by Licensed Physical Therapy Assistants and determine appropriateness, as well as modify treatment plan as indicated.

  • Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment.

  • Complete all required documentation according to guidelines specified in Documentation Manual.

  • Maintain professionalism and follow all national/state associations regulations at all times.

 

QUALIFICATIONS:

  • Must have a degree in Physical Therapy from an accredited program.

  • Active state license required.
